Abstract

56-year-old male patient with endodontic tooth fracture 36. We propose an atraumatic extraction treatment of the piece by performing a dental section that allows the least possible pressure to be exerted on the walls of the alveolar bone to preserve its architecture as much as possible. Since the mesial root socket shows slight bone loss in the vestibular area, we decided to place the implant in the distal root socket and preserve 100% of the inter-root septum structure. We obtain a primary stability greater than 40Nw and we place the healing abutment. We regenerate the mesial alveolus with a xenograft.

Rehabilitation of the anterior maxillary aesthetic sector and posterior mandibular sectors

Abstract

A 44-year-old male patient presented with edentulism in the anterior upper arches and posterior lower arches. Carrier of removable prostheses, shows great resorption in the anterior maxillary sector. We propose different treatment options, among which we propose horizontal regeneration implant placement using xenograft together with autologous bone and resorbable membranes.


One of the characteristics of the Vulkan implant is that it is created from a type IV titanium with which a cold forming process is applied, which gives it resistance and elasticity characteristics that are close to grade V but maintaining the Osseointegrative capabilities of class IV titanium. Thus, we used 2 implants with a diameter of 3.3 mm in the anterior sector with the confidence that they will support (together with a 3.75 mm implant) a screw-retained prosthesis. The implants in the posterior sectors are 4.2 mm in diameter, thus respecting the measurements recommended by our company for the pieces that we are rehabilitating. We load all implants at 4 months because we have performed bone regeneration techniques in the anterior sector.

Study Summary

We present the clinical case of a 58-year-old woman, without systemic pathology of interest, non-smoker, with a cemented metal-ceramic fixed prosthesis in the upper arch on 9 abutment teeth with different degrees of periodontal involvement and restorative failure. After clinically and radiographically evaluating the case, a consensus is reached with the patient, perform the extraction of the metal-ceramic prosthesis and the 9 remaining abutment teeth in the upper arch, place 8 immediate Vulkan® External Hex implants in the residual alveolar processes and proceed with immediate loading using 1 mm high Multiuse® transgingival abutments and PEEK® abutments for straight implants. The key points in the technical execution of the case focus on a minimally atraumatic extraction of the abutment teeth for the remaining alveolar process, a correct drilling sequence, an optimal primary stability of the Vulkan® External Hex implants and an excellent passive fit between implant connection and prosthetic attachments.

Se presenta el caso clínico de una mujer de 49 años de edad, sin patología sistémica de interés, fumadora de 10 cigarrillos/día, portadora en arcada superior de una corona metal-cerámica en incisivo lateral superior izquierdo (12#). La paciente presenta en el diente 22# desprendimiento de la corona metal-cerámica y dolor intenso a la palpación en zona periapical. A la exploración radiográfica y clínica, se diagnóstica fractura oblicua del diente 22# y se consensua con el paciente la extracción de dicho diente y la colocación de un implante Conical Connection X-Body de Vulkan® 2 mm subcrestal en dirección axial y realizar una restauración inmediata mediante el uso de un aditamento transgingival Multiuse® de 3,5 mm de altura y un pilar para implante recto PEEK®. Los puntos clave en la ejecución técnica del caso se centran en una extracción de los dientes pilares mínimamente atraumática para el proceso alveolar remanente, una correcta secuencia de fresado. Los implantes Conical Connection X-Body de Vulkan® presentan una extraordinaria estabilidad primaria y asimismo, su diseño permite un posicionamiento marcadamente subcrestal con el que resulta mucha más predecible combinar recursos mucogingivales (injertos subepiteliales) y/o procedimientos óseo-regenerativos. El ajuste pasivo excelente entre conexión implantaria y los aditamentos protésicos garantizan una fácil obtención del provisional inmediato directo en boca.
